How to Order:

Shipping is not included in price listed.
Add 10% of subtotal for shipping to addresses East of the Mississippi River.
Add 15% of subtotal for shipping charges to addresses West of the Mississippi River.


     3.  Enclose your completed Order form & Check or Money Order to Shades of Oak and mail to:
